Mahesh Babu didn't undergo a hair transplant, he is wearing a hairpiece/weave (modern version of a wig).
why I think that he is wearing a hair piece?
His natural hair is all curly and he used to keep it short look at all pictures of his childhood and also in 'Pokiri'
He is not able to style his natural hair for the same reason and couldn't give many variations in the characters he plays.
His current hair is having too much of density and it is very unnatural for someone to have that density in the ’40s, you won't even see his scalp in any of his movies.
